New Student & Kindergarten Registration for 2009/2010 School Year

Who needs to register?

• Students new to the district.
• Students who did not attend Mason this year.

Where do I register?

• At the school office

 When?

• Thursdays between 9:00a.m - 2:30 pm.

• Please come in to register as soon as possible.  Student will not be assigned to a teacher until registration packet is complete. School will be closed during summer vacation.  Thus, come in before July 21st.  See you soon!

You need to bring:

• Birth Certificate (original certified copy) - Child must be 5 years old on or before Dec. 2nd
• Immunization Record (original)
• Proof of Residence—Document must show both your name and address.  i.e., A recent utility bill (SDG&E, phone, water, cable), a rental agreement, property tax payment receipts,
  mortgage documents, r military housing orders.

KINDERGARTEN IMMUNIZATION REQUIREMENTS

• Polio: 4 doses, but 3 doses meet requirements for ages 4-6 if at least one was given on or after the 4th birthday
• DTP: 5 doses, but 4 doses meet the requirement for ages 4-6 if at least one was on or after the 4th birthday
• MMR: 2 doses both on or after the 1st birthday
• Hepatitis B:  3 doses
• Varicella:  1 dose or health care provider-documented varicella disease or immunity

The CHDP physical, which is required for entry into 1st grade, can be completed anytime after March of this year for Kindergartners entering in September.
